APPLICATION AND RELEASE FORM

Legal things you and your family should know about.

Event Description: Participants will take part in World Vision’s Leadership Retreat Program. Invited because of their leadership skills and community involvement, 15-20 youth will have the opportunity to take part in a weekend retreat. After arriving at the location on Friday afternoon, participants will have the opportunity to meet with other young leaders from across their province and receive training in Global Issues from World Vision staff. On Friday and Saturday evening the participants will stay overnight, sleeping in male and female cabins at the location. Male and female World Vision staff will supervise the participants throughout the weekend. On Saturday participants will receive leadership training and participate in outdoor learning activities (Including: team-building and interactive games) led by certified staff. World Vision Canada will cover for all of the costs associated with the weekend retreat[1]. We expect the event to be finished by approximately 2:00 PM on Sunday.

The undersigned acknowledges and voluntarily assumes all risks associated with participating in the World Vision Leaders Retreat Weekend. The undersigned hereby waives for themselves, their heirs, executors, administrators and assigns, any recourse he or she may have against World Vision Canada and its directors, officers, volunteers, organizers, employees, sponsors, successors, assigns and agents, and releases and discharges such persons from all claims, demands, damages or actions, and absolves such persons from all responsibility for any injuries that may arise in any way in connection with the World Vision Leaders Retreat Weekend.

We further hereby undertake to hold and save harmless and agree to indemnify all of the aforesaid from and against any or all liability incurred by any or all of them arising as a result of or in any way connected with my participation in the World Vision Leaders Retreat Weekend.

We also agree to the potential recording and use of the Participant’s image and/or comments. World Vision Canada and its authorized partners and licensees may use, reproduce and communicate footage, recordings, images and/or comments of the Participant identified below required during the World Vision Leaders Retreat Weekend, in whole or in part.
